If your son or daughter needs a physical evaluation for the 2020/2021 year, individual sports physical exams are available through the Center for Sports Medicine in Walnut Creek. After you have completed the registration, you will be provided additional details and instructions on how to schedule an appointment.
This registration session will allow you to complete contact information, sign up for an Athletic Physical for Northgate Athletics, and submit payment. Please read through each page carefully to ensure that all information is provided accurately. To complete registration through our secure site, please have your Visa, MasterCard, Discover, or ACH Bank Account information available.
Registration will be open for one month, then the registrations will become closed. Once the registration window has closed, Glen Barker will reach out to that student-athlete via phone or email to confirm and provide details on how to schedule your son or daughter's individual physical at The Center for Sports Medicine, located in Walnut Creek. After the sports physical is completed, please upload the physical form to SportsNet.
